Assessment Guidelines

For Adult Patients:
  • Use 0-10 numeric rating scale
  • Ask "On a scale of 0 to 10, how would you rate your pain?"
  • 0 = no pain, 10 = worst possible pain
For Pediatric Patients:
  • Use FACES scale for ages 3 and older
  • Show faces and ask which matches their pain
  • For infants, use behavioral indicators
Documentation:
  • Record initial pain score
  • Re-assess after interventions
  • Note location, quality, and duration
