Assessment Guidelines
For Adult Patients:
- Use 0-10 numeric rating scale
- Ask "On a scale of 0 to 10, how would you rate your pain?"
- 0 = no pain, 10 = worst possible pain
For Pediatric Patients:
- Use FACES scale for ages 3 and older
- Show faces and ask which matches their pain
- For infants, use behavioral indicators
Documentation:
- Record initial pain score
- Re-assess after interventions
- Note location, quality, and duration
